Dr Keerthi Talari is a Consultant Rheumatologist and Clinical Immunologist at Yashoda Hospitals, Secunderabad, with over 14 years of experience in managing a wide range of autoimmune and musculoskeletal conditions. She specialises in the treatment of systemic lupus erythematosus, rheumatoid arthritis, vasculitis, scleroderma, osteoporosis, and other complex rheumatological conditions, and is known for her patient-focused, evidence-based approach to care. Dr Talari holds advanced qualifications in rheumatology and has been recognised with several academic awards for her clinical and research excellence.

What inspired you to pursue a career in rheumatology?

Rheumatology appealed to me because it embodies the true spirit of internal medicine—systemic, complex, and intellectually engaging. It allows us to understand disease across multiple organ systems rather than in isolation. At the same time, there is a significant gap between the number of trained rheumatologists and the large burden of patients with autoimmune diseases, particularly in India. This combination of clinical depth and unmet need motivated me to pursue rheumatology, where I felt I could make a meaningful and lasting impact on patient care.

How do you balance clinical practice, research, and personal development?

Balancing these domains requires conscious structuring rather than perfect equilibrium. I prioritize patient care during clinical hours while dedicating protected time for research and academic work. I integrate learning into daily practice—each patient encounter becomes an opportunity to refine knowledge and generate research questions. For personal development, I actively pursue new challenges, including exploring generative AI, music, and painting, which help maintain creativity and perspective. Above all, spending meaningful time with my daughter beyond work hours is my highest priority, as it keeps me grounded and provides true balance.

What current innovations in systemic lupus erythematosus excite you the most?

Among recent advances, CAR-T cell therapy is particularly exciting. It represents a paradigm shift from long-term immunosuppression to the possibility of targeted, sustained immune reset. Early studies suggest that deep B-cell depletion can induce durable remission, potentially redefining how we approach refractory lupus. While still evolving, this strategy holds promise not only for improving outcomes but also for moving closer to a functional cure in selected patients.
