Psoriatic arthritis is a chronic, progressive, inflammatory form of arthritis, which develops in up to 30% of people with psoriasis and is associated with increased risk of systemic inflammatory disease. Treatment options include nonsteroidal anti-inflammatory drugs (NSAIDs), corticosteroid injections, disease-modifying antirheumatic drugs (DMARDs) and biologic therapies, including inhibitors of tumour necrosis factor (TNF) α, interleukin (IL)-12, IL-23 and IL-17. Recently approved therapies include the Janus kinase (JAK) inhibitors, and other promising agents in development include tyrosine kinase inhibitors. Future challenges include personalised medicine, earlier diagnosis and achieving a low-level of disease activity.

Overweight and obesity are common comorbidities in psoriatic arthritis (PsA) and can negatively affect disease activity and treatment response. The phase 3b TOGETHER-PsA trial evaluated the concomitant use of ixekizumab and tirzepatide in adults with PsA and overweight or obesity. In this interview, conducted at EULAR 2026, Dr Laura Coates (University of Oxford, Oxford, UK) discusses the rationale for this combination approach, highlights the study’s key findings, and considers their potential implications for the future management of PsA.
Long-term dual IL-17A/F inhibition in PsA and axSpA.
The US Food and Drug Administration (FDA) has approved deucravacitinib as the first tyrosine kinase 2 (TYK2) inhibitor for adults with active psoriatic arthritis (PsA), marking a new targeted treatment option for this chronic inflammatory disease. In the phase 3 POETYK PsA-1 and PsA-2 trials, the once-daily oral therapy significantly improved disease activity compared with placebo, with over half of treated patients achieving an ACR20 response at Week 16. By selectively inhibiting TYK2 signalling pathways involved in psoriatic disease, deucravacitinib offers a novel mechanism distinct from traditional JAK inhibitors, expanding its role beyond plaque psoriasis and introducing a new oral option for patients with PsA.
